Checklist of Eligibility Requirements for Bidders

• Department of Trade and Industry (DTI) business name registration or SEC Registration Certificate, whichever may be appropriate under existing laws of the Philippines, supported with the necessary information using the prescribe forms
• Valid and current Mayor's permit/municipal license
• BIR Registration Certification, which contains the Taxpayer's Identification Number
• Statement of the prospective bidder that it is not "Blacklisted" or barred from bidding by the Government or any of its agencies, offices, corporations or LGU's, including non-inclusions in the Consolidated Blacklisting Report issued by the GPPB or CIAP
• Articles of Incorporation, Partnership or Coorporation, whichever is applicable, including amendments thereto, ig any
• Sworn affidavit of the bidder that it is not related to the head of procuring entity, members of the BAC, TWG and Secretariat and members of the PMO, and the designers of the project, by consanguinity or affinity up to the third civil degree
• Vali joint venture agreement, in case of a joint venture
• Letter authorizing the BAC or its duly authorized representative/s to verify any or all of the documents submitted for eligibility check
• Certification under oath that each of the documents submitted in statisfaction of the eligibility requirements is an authentic and original copy, or a true and faithful reproduction of the original, complete, and that all statements and information provided therein are true and correct
• Other appropriate licenses as may be required by the procuring entity concerned

• Statement of ongoing and similar completed government and private contracts within the period specified in the IAEB, including contracts but not yet started
• Copies of end-user's acceptance letters for completed contracts
• Specification of whether or not the prospective bidder is a manufaturer, supplier or distributor

INVITATION TO BID

Prospective bidders should posses a PCAB License applicable to the contract, have completed a similar contract with a value of at least 50% of the ABC, and have key personnel and equipment (listed in the Eligiblity Forms) available for the prosecution of the contract. The BAC will non-discretionary pass/fail criteria in the Eligibility Check/Screening as well as the Preliminary of Bids. The BAC will conduct post-qualification of the lowest calculated bid.

All particulars relative to Eligibility Statement and Screening, Bid Security, Performance Security, Pre-Bidding Conference, Evaluation of Bids, Post-Qualification and Award of Contract shall be governed by the pertinent provisions of R.A.9184 and its Implementing Rules and Regulation (IRR).

The BAC will issue to prospective bidders Eligibility Forms at Provincial Engineer's Office, New Capitol Bldg., Siquijor, Siquijor, upon their submission of a LOI, and upon their payment of a non-refundable amount of Three Hundred Pesos Only (P300.00) to the Siquijor Prov'l. Gov't. Cashier. Prospective bidders shall submit the Eligibility Requirements to the BAC at the said address. They may also obtain the results if the Eligibility Check by the BAC at the same address.

The BAC will issue Bidding Documents only to bidders declared by the BAC to be eligible for the bidding upon payment of a non-refundable amount based on the BAC Tariff to the Siquijor Prov'l Gov't. cashier.

The Prov'l. Gov't. of Siquijor assumes no responsiblity whatsoever to compensate or indemnify bidders for any expenses incurred in the preparation of their bids.
